    Coordinated family supports

    Empowering adults with developmental disabilities

    to live independently in the family home


    What we Do

    • Identify and provide support for residing successfully in the family home


    We assist with in home risk management, applying for IHSS, applying for SSI, household safety coaching, anger management coaching, and ensuring families have access to all generic resources, such as, Food Banks, WIC, low cost utilities, etc…



     

    • Train the participant and family to navigate tailored services and overcome access barriers


    We assist the family with navigating the service provider selection process.  Whether they need a day program, employment services, or social recreational programs, we walk the family through the entire process from making recommendations to services, to scheduling interviews/walkthroughs, and advocating for the participant when choosing a service provider




    • Assist with scheduling services, including medical and other appointments


    We assist participants and their families with scheduling appointments.  Whether it be for a doctor appointment, Department of Motor Vehicles, SSI appointments, we will walk the families through the entire process




    • Offer future planning, especially for participants with aging caregivers


    We assist families with planning for the unforeseen.  From assisting with conservatorship, special needs trusts, to facility placements, we help families prepare for the unthinkable. 




    • Connect participants seeking employment with the Department of Rehabilitation and/or job supports


    We assist participants that are looking for employment by finding resources, assisting with job/interview preparation, and skill training so the individual can excel in a competitive work environment




    • Ensure consistent training across providers to meet specific participant needs


    We work with all Regional Center vendors that provide services to the participant in order to ensure all IPP goals are met and to the participant's specific needs




    • Arrange backup support and provide it when primary plans fail


    We ensure that the participant has backup supports and providers and will provide support as needed, including direct support
